Membership Cancellation Policy
A minimum of thirty (30) days written notice is required for cancellation of the membership agreement. Cancellation of the Membership Agreement can be done at any time during the membership period and shall be effective on the first day of the month following the expiration of the thirty (30) day notification period. Cancellation shall not relieve the Member of his/her responsibility to pay any outstanding fees of membership or outside services relating to a claim not covered in the Agreement, before the effective date of cancellation and any fines assessed against the Member for late payment(s). Please understand in the event your membership termination request falls within the 12 Month Contract period, this request will be honored the same as Month-by-Month Contract Period to take effect 30-days after the notification period and a prorated refund will be provided.
By terminating your membership(s), the member will forfeit any/all initiation fees paid and the member will be required to pay the then current Initiation Fee(s) at the time of rejoining. All outstanding dues, charges and fines are immediately due and payable. All cancellation requests must be made, in writing firstname.lastname@example.org. Termination by telephone or fax is not permitted.
